Pharmacy script report ?

Is there such a report or template for a pharmacy script ? Ideally that would be pre-populated with patient information, client address, etc.

Hi,

Attached is our prescription template. Feel free to use and this can be uploaded for community use as well. You will need to alter to add in your letterhead and clinic details (we put these in the footer). I upload as a patient letter.
