New Dose Calculations

Currently the medication dose calculations only support 

Dose (unit/kg) x weight (kg) to give a Unit / dose calculation

Suggesting

Add support for different mathematics  specifically BSA dose calculations.

One problem is that different groups use different BSA coeffecients - ie we dont all agree on the exact equations.

How are they used in practice? Quick googling suggests they are based on weight multiplied by a coefficient which is species dependent. Once you have the BSA, how is the dose determined?
