The Customer Information page lists the patient relationships that a customer has in 3 columns, "Name", "Description" and "Details". The "Details" column is a string that displays the relationship type, from date  and to date e.g.: Patient Owner(28/01/2008 - 01/06/2012)

To support sorting on the From and To date, the "Details" column should be split into 3 columns, "Type", "From Date" and "To Date".
