Improve document support during invoicing -----------------------------------------

During invoicing, document acts are created for each template associated with a product's document node. These are linked to the invoice items documents node, and also to the patient's act.patientClinicalEvent.

This needs to be improved so that: * parameters in act.patientDocumentLetter templates are prompted for. * documents can be added, edited and deleted. The product and patient nodes will be hidden. The clinician node will default to that of the item's clinician node

NOTE: There is no requirement to: * version documents associated with an invoice item * retain documents if the product changes
