If a patient's medical history includes act.patientClinicalProblem records, any child records are also linked to the problem's parent act.patientClinicalEvent. The patient medical record summary view displays these linked acts twice - once as a child of the event, and a second time as a child of the problem. The relationship between problem and child record should be ignored

To reproduce: * create a new act.patientClinicalEvent, and add an act.patientClinicalProblem to it. * In Patients -> Medical Records -> Problems, edit the act.patientClinicalProblem and add an act.patientClinicalNote * Go to Patients -> Medical Records -> Summary. The act.patientClinicalNote will appear twice
