Add "New Visit & Note" button to Medical Records ------------------------------------------------

The most common use of medical records is to add notes. If there is a note added without a visit (eg.Phone call etc) a Visit must first be created.

There was a request for a new button in Medical Records that allowed a user to create a New Visit & Note with one click.

i. The new Visit inherit the date from the Note. Default should be the current date. ii. The Visit Reason will be left Blank. iii. The focus should be in the Note box. iv. The button should live in the Medical records space with New, Delete buttons etc. There is no need for a current selection to be current for this button to be visible.
