We have just started using a new pathology lab and I forged their submission form. If anyone else is using Vepalabs, feel free to edit and use this document if you want.

This form mimics a standard ACE Laboratory Services submission form and can be generated with an investigation.  The printed form will contain pre-filled practice, client, patient and Requisition ID details.  Prints to A4 page.  Tick boxes are as images, for the paper form only, at this stage.

Insurance companies often want a cruciate exam before accepting a pet for insurance.

This form prints client, patient and veterinarian information with all questions as found on insurance company-supplied forms.

This simple form prints out the data you need when identifying a patient when performing an in-house IDEXX VetLab Station test.  It is arranged exactly as you see on the input screen.

Attach this form to your in-house investigation. When you bill an in-house test, the investigation will appear. Click ok then "Apply" in billed item.  Then click on Invoice item to open investigation window and click "Print Form".  Prints to an A4 page.
