Is it possible to have the link to the csh invoked via some method other than a keyboard shortcut, eg. a link or button?

Touchscreen devices (eg. iPad) do not have an F1 button and even if you used some other keyboard combo the keyboard is not presented unless you touch some input box. 

I'm not sure how you would do it? Often the help menu is in a layer that is not accessable such as when you have a dialogue open. 

I've often thought it would be useful to have the new window button accessable at all times as well. Perhaps there could be a toolbar at the top which has a very high z-index?

What's the iPad convention for launching help?

I could add a [?] button next to the close button [x], but it would need to be spaced far enough apart to prevent pressing one instead of the other. Might look a bit odd.

The new window button isn't accessible from modal dialogs. I don't think raising the z-index will change this; it would need to be a child of the modal dialog I believe.

I don't really think there is a convention. The only cloud service I use which has something similar is xero. It has a similar help menu to open which has a submenu item something like "get help for this page".

I don't really think that the buttons being close to each other in the modal window would be an issue Tim. There are plenty of pages and apps that have close links or buttons and it never really seems to be a problem.

I think there needs to be some sort of link or button as most new users won't know that a keyboard shortcut exists. I'm pretty sure that most of my staff don't even know what a keyboard shortcut is. I've tried at times to get people to use them as I am 10 times faster when using the keyboard shortcuts but they never end up using them.
