Online patient consent & admissions forms (like pre-flight online boarding pass check-ins)

We would love to have an integrated system for client consent forms for surgery, dentistry, procedures, boarding and grooming.

I would envisage this like the pre-flight boarding pass check-in with Qantas. 

We could do a basic version of this via a Captcha form from our website.

Ideally the information would be automatically saved to OpenVPMS.

There would still be a requirement for some face to face admission procedures, but this would streamline it, professionalise and halve the time taken. It would also be an opportunity for an owner to review vaccination or parasite control status with the family in the comfort of their own home the day before admission. (How many clients can't remember the name of the box of tick & flea preventatative sitting in the laundry, or whether pets are up to date, because their partner takes care of the animals?).

Open to suggestions, what do others think?

Great idea. I dont have much time and am hopeless with software but if you want some assistance let me know. 

 

Also: I would like to print out a hospital form with patient details and owner details to have on cage - is there something like this somewhere. I am still on ver 2.02 but as soon as I can afford it will upgrade.

Further to previous post, we are implementing an online / mobile Capcha form with pre-groom check in information, for owners that wish to save time at admission. If successful we may roll this out to boarding admits, dentals and possibly surgery too. (There would still need to be some face to face check in time, but could be accelerated). Ideally these would pre-populate regular admission forms.
