This issue can occur if: # an invoice item is backdated; and # the most recent visit is COMPLETED; and # the visit is dated a day or more after the invoice item
In this case, a new visit will be created with the same date as the invoice item. Each time the backdated invoice item is saved, a new visit will be created.

Another circumstance where multiple visits are created (commonly in our hands): 1. A medical record has only COMPLETED visits (clinician may have manually changed to complete during consult or entered medical records directly). 2. An invoice is created. 3. For EACH item billed to that patient, a new visit is created. If the item has no label/doc associated, the visits appear as blank COMPLETED visits.
I suspect this may have happened in Amandas case.

Sorry about the barrage of comments but I have just been testing and have the following further details including a correction:
1. It seems only items that have associated documents/labels create the visits. 2. If the invoice is created today, multiple visits are only created if there are no visits with the same date.
So, for example; 1. A medical record has only COMPLETED visits with none matching the date of the invoice being created (clinician may have manually changed to complete during consult or entered medical records directly). 2. An invoice is created. 3. For EACH item billed to that patient with a label/doc, a new COMPLETED visit is created.
4. If then another invoice is started or more items are added, then they are added to the last COMPLETED visit and no new visits are created.
