This template can be linked to a urinalysis product and investigation type.

It can then be printed and used to record the results and then be scanned and linked back to the investigation request. It requires version 1.4 +

Version 2.0

Version 2.0 requires no printing. It can be previewed which produces a pdf form. The form can be completed using a pdf reader and then printed to a pdf to save on paper. It is important to note that saving the pdf after you fill in the form will not save the completed form- you need to print it to pdf.

Version 2.0 also has fields for all tests on a 10 test dipstick.

Printing to pdf

• To be able to print to a pdf on windows I recommend you install the open source project PDFcreator which installs a pdf printer (it's available from sourceforge and is free).
• In OSX use the save as pdf option from the pdf menu
• On linux print to file.
