Digital new client form populates a new patient file in OpenVPMS

There is an opportunity to leverage VetCheck's new client form to be integrated into OpenVPMS so that on completion it creates a new patient file automatically. 

When the VetCheck new client form is emailed to the client prior to the visit, the client completes the form on any digital device, and a new patient file is generated within OpenVPMS with the following details:

Client first name
Client last name
Client email address
Client phone number
Client address
---
Pet name
Pet species e.g. dog, cat, horse.
Pet breed e.g. French bulldog.
Pet color e.g. Brindle, Black & White, Chestnut.
Pet sex e.g. Male, female, unknown.
Pet desexed status e.g. Entire, neutered.
Pet date of birth e.g. 11-12-19, 19-12-11, or 11 Dec, 2019.
Pet weight.
Pet microchip number.

Vaccination name or type e.g. C3, C4, C5, F3.
Last vaccination DATE GIVEN e.g. 11-12-19, 19-12-11, or 11 Dec, 2019.
---
Appointment id e.g. 12345678.
Appointment date e.g. 10-02-22 9:30am.
Appointment reason e.g. Dental, vaccination
Pet history e.g. Description of previous health concerns/issues

This is currently available with other practice management system integrations and can help teams save a lot of time in onboarding new clients, curbside appointments and particularly in emergency situations. 

We are seeking feedback/interest level from the OpenVPMS community.

We would be interested in this if it means the client can complete the form on their own devices or at home.

My comments are below after each line. Some of it will be different from us as we are an emergency/specialist. Ideally can we chose what is *required and what isn't in our own set up?

Hi,
Thanks for your feedback. You can customise your new client form however way you want. The only required fields are the client first name, client last name, email address and pet name.

All other fields are optional. The above list mentions all optional fields that could be pre-filled into the new client file of OpenVPMS.
