History Order

I am currently in the process of mining records for ASAVA re-accreditation and am finding that trying to present histories in a readable format for inspectors is difficult. If you view the attached file you can see that the entry dates are all over the place.
Can someone tell me the reason for this - is it to do with completion dates versus in-progress - in other words is there a need for us to change the way we are entering records or is there a bug in the system??

Looks like the first visit was created during the initial consultation and not completed as obviously the patient went into hospital. It would seem the second visit was created manually (i.e. not via workflow) through medical records workspace on the afternoon of the 21st. Not sure why ?
Both visits status changed to completed on the 22nd as seen by visit completion date.
I haven't heard of an issue in 1.3 where workflow created multiple in progress visits unless the previous in progress visit was dated over a week before the current date.
