Hey Jocelyn, On the bottom of Patients->Medical Records is a "Print" button. You can certainly modify the patient history print template (patientClinicalEvent.jrxml) to include a first page header. If you are confident in using Ireport you can do it yourself otherwise your implementor can do it or myself or someone in the forums.
