Setting up remote access on a new laptop

I tried to put this up this afternoon, but now I can't find my post, so apologies if this pops up twice later on!
I have replaced an old, recently deceased laptop. I have set up the remote access program on windows 7 with the 198.whatever number for the clinics login, the same as the last laptop. I can use the laptop remotely inside the building at work, obviously via the wireless network in the building, but can't do it from home over the Internet.
What am I doing wrong?

Hi Gary,
The public Internet does not have direct access to your internal network (which is a good thing). You need to get your local IT company to advise you on how to setup a VPN connection on your laptop tothe clinci at which point you will be able to log in via remote desktop or access OpenVPMS directly from your browser.

Within your building, you are probably using your in-house wifi connection from laptop -> wireless router/modem -> server, hence the 198.n.n.n log in address.
In the outside world, your laptop wifi doesn't work: you need a connection via internet. I actually use logmein.com free to log into my server from my laptop: worked for me even from the US. Your laptop screen actually then displays the server screen.
